髖臼周圍惡性腫瘤的外科治療

摘要目的：透過對本組11例髖臼周圍惡性腫瘤外科治療分析，探討髖臼周圍惡性腫瘤的治療方法。方法：2005年3月～2008年6月髖臼惡性腫瘤手術治療的11病例，軟骨肉瘤4例、惡性纖維組織細胞瘤2例、轉移瘤2例、鉅細胞瘤2例、骨肉瘤1例。5例手術刮出病變、球磨鑽打磨，無水酒精滅活、植入複合骨；2例半骨盆截肢;2例曠置術；2例人工假體置換術。結果：11例隨訪8～33個月，2例刮出植骨病例區域性復發，其餘病例術後恢復良好，功能滿意。結論：刮出植骨區域性複發率較高，恢復時間長；股骨頭曠置肢體短縮，跛行明顯；人工假體置換可最大限度地保留肢體功能；半骨盆截肢可緩解症狀改善生活質量。根據患者的病情、狀態、意願、經濟條件對患者進行綜合評定，選擇合理術式至關重要。遼寧省腫瘤醫院(中國醫科大學腫瘤醫院)骨軟組織腫瘤科鄭珂
